b. On the Registration Form, click on the "Print" button to print your form.

d. Come back to this internet page and either:
** Pay by credit card by clicking on the PayPal button below and selecting the appropriate option(s), OR
** Pay with a check by mailing a copy of your printed form and your check in the appropriate amount to:
